Cancellation Policy

Liability Waiver for Participation in Yi Ren Medical Qigong Class Acknowledgment of Risks: I, the undersigned, hereby acknowledge that I am voluntarily participating in the Yi Ren Medical Qigong class offered by certified Yi Ren Medical Qigong instructor Amy Putiri. I understand that the practice of Medical Qigong involves physical activity that carries inherent risks, including but not limited to, injuries related to physical exertion, existing health conditions, and the potential for falls or other accidents. Medical Clearance: I affirm that I have consulted with a physician regarding my participation in this class and that I am in good physical health, or I have disclosed any medical conditions that may affect my ability to engage in the class activities. I agree to follow all guidance provided by the instructor and to inform them of any discomfort or concerns during the class. Waiver of Liability: In consideration of my participation in the Medical Qigong class, I, on behalf of myself, my heirs, assigns, and personal representatives, do hereby release and hold harmless Amy Putiri from any and all claims, demands, and causes of action arising from or related to my participation, including any injury or damage arising out of my actions or negligence, whether within or outside the class setting. Indemnification: I agree to indemnify and hold Amy Putiri harmless from any claims made by third parties as a result of my participation in the Yi Ren Medical Qigong class. Registering for this class gives consent to use your registration as your signature. I have read this Liability Waiver, fully understand its terms, and acknowledge that I am giving up substantial rights by registering for this class. I am registering for this class freely and voluntarily, without any inducement.
